Please review our handbook for our Faith Formation policies. If you would prefer a printed copy of the handbook, contact our office and we will provide one for you.
Registration Forms
Families need to fill out one family form, and one additional medical waiver per student. You can return these by mail or to our main office.
Payments
No family will be denied access to the Faith Formation program because of their inability to pay the fee. We do have some limited sponsorship funds available to cover fees for those in financial need. If you are able, please consider making a payment into the sponsorship fund to bless a fellow SMM family!
Fees:
- One child: $75
- Two children: $140
- Three (or more) children: $200
If you are volunteering as a catechist/classroom aid and are enrolling your own children in the program, please enjoy a reduced rate of $50 for your family.
There is an additional sacrament fee of $20 for First Communion and $25 for students who are receiving Confirmation during the year.
Payment methods:
- Check (can be mailed or brought to the main office)
- Cash (please do NOT send cash through the mail! Cash is accepted at the parish office; you will get a receipt.)
